A New Online Patient Experience!
On October 24, 2023, Siouxland Community Health Center introduced Epic, a new electronic health record (EHR) which gives you access to MyChart.
MyChart allows you to take control of your health information and manage care for yourself and your family members
MyChart is easy to use!
- Message your provider
- View your health records
- Check in online before your appointment
- Request medication refills
- Pay your bill
- View your statements
Sign Up for MyChart:
Signing up for an account is easy and, in most cases, only takes a few minutes!
(If you signed up for the patient portal in the old system, you will need to set up a new account with MyChart.)
- Click the “Sign Up For MyChart” button above or download the MyChart mobile app to your phone or tablet.
- If you do not have an activation code, choose the “Sign Up Online” option.
- When using the MyChart mobile app, search for Community Health Care, Inc. and add CHC as your organization.
Billing:
- Please use the following link for online bill pay for SCHC services billed before October 24, 2023: Pay My Bill
- Billing statements for all SCHC services received on or after October, 24, 2023 can be paid using the MyChart patient portal by clicking HERE
